Post form info

What I need is to have a form on a page in my site that when submited will post itself into a topic of the forum. 
Each form would also have a file or two that would need to be saved as an attachment to the post.

Also, the people who submit the forms wouldn't be able to view the board that it is posted in.

I can make the form but I have no idea about any of the rest of it.
