Is there a list with the modifications that are used on this site?

I seem to recall the mods used on this site are customised versions of:

* OpenX ad server (not provided as a mod anywhere)
* Topic Solved
* Wiki Links
* SimpleDesk
* Search Focus Dropdown

There may be a few others but those are the only ones I can actively remember ever seeing.

And the 2. (or "first") menu at the top?

That's not a mod, that's just custom hacked into the template. Truth be told I'd actually forgotten about that.

Thought there was a mod that also put a menu at the top similar to what is on SMF, but I searched and could not find it.

Yes, you're right, though the menu mod came after it was done here first.

In any case, it's been a while since I worried about the modifications here other than SimpleDesk... :/

If the SMF logo bothers you, then replace it with on of your own...

I believe that's just a template edit, unless it's been re-done since I last had to deal with it.
