You cannot download or install new packages because the Packages directory....

When I try to install packages more often than not I get this error:
You cannot download or install new packages because the Packages directory or one of the files in it are not writable!
Somtimes it works somtimes it does not. I would like to figure out why so I wont have any problems installing mods.
The server is a windows 2003 server. I am RDP'ed into the server and am looking at the folders permission and they are indeed writeable

Yes they are. The only thing that complicates it is there is a Plesk Control Panel for managing our website. There is a feature called Check permissions on Domain that goes through every file/folder in the entire website and makes it writeable.
Also does it matter is users are logged in when I apply the mod?

It might be worth contacting your host to confirm that you are following the correct procedure for setting permissions. Also I would not recommend setting every file/folder on the entire website to writeable, as it's potentially a security problem.
You can install modifications whether there are users logged in or not. If a package requires the forum to be offline (SMF upgrade packages), it will automatically set the forum to maintenance mode while installing, and then put it back online once installed.

I believe that this is one of the things that could be cured by creating a directory named "temp", inside your "Packages" directory.
That needs to be read/write-enabled.
