Go to admin >> Click on the Security and Moderation >>>> Click on Anti-Spam >>>> Select Require verification on registration page and Require verification on all guest searches >>> at the same page check Configure Verification Methods . Select a method of verification from there .

http://www.stopforumspam.com/ is another good mod.
Sign-up and get the code.
Place it in the mod settings for SFS.
And then when you register with any listed e-mail addresses in http://www.stopforumspam.com/, you'll get an error message.

The Stop Forum Spam mod I feel is the most important mod you can add to a forum

Once installed its best to have it only filter on IP address and Email. If you have it filter by Username you risk blocking non spammers from joining your forum and creating an angry new member.

A high level of distortion on Captcha will help block many spambots.

A couple security questions on registration blocks the bots too. Are you a bot y/n, What year is it? etc.

Once you have those things in place spam will be a very rare thing on your forum. We pretty much just sit back and watch the bots hammer away all day every day. They are relentless.
