Can I see which admin added a ban?

Browsed through the admin panel, didn't find it.

Would have thought adding any ban triggers would appear in the Moderator log. Have you looked there?

Some logs have to be enabled before you can use them.

So, did you enable the Moderation log? ;D

Admin > Core Features, somewhere in the middle. Assuming you are running 2.x, of course.
