Members can't register - database error

Hello,
Whenever a member registers he gets this error
From error logs, the error is:

What I have done to solve?
I read different topics on the same question, but they weren't helpful to me.
I tried replacing the subs-members.php with a fresh one, but didn't help me.
I was on smf 2.0, i upgraded the forum to the latest 2.0.2, but it didn't help me as well.
After up-gradation, I only have aeva media mod installed, not any other.
P.S
Whenever someone registers, he get this database error on screen, but i check from admin panel that his registration gets in "awaiting activation" queue (as I have email activation activated), but he don't gets an email from my forum for activation.
I had a lot of spam members registration, which i pruned and now I have disabled registration until i solve this problem.

Replacing files isn't going to do anything to sort-out problems with the database, halik, whatever you do with them.
Two obvious things to try:
1-Go to Admin>Forum maintenance and attempt to "Find and repair any errors".
2-Go to your CPanel>databases>MySQLDatabases and attempt to repair the database.

1. I have done the "find and repair any errors", but still the same problem.
2. Everything is working fine, besides this problem, let me know what to do from database.

You'll see "Modify Databases", with the name of the database.
You'll, also, see "Check DB" and "Repair DB".
If that doesn't find and repair the problem, you'll need to BACKUP THE DATABASE! Then, go to phpmyadmin to drop the duplicate entry.
Just a hunch, though...
Try going to Admin and disabling things like "Quick reply" and "WYSIWYG" and see what happens.
If everything works OK, try enabling them, again.
If you have a mod which affects Quick Reply, try disabling that, too.
