Error after trying to install a mod

I tried to install a spoiler mod (http://custom.simplemachines.org/mods/index.php?mod=195) after our previous spoiler stopped working. I de-installed the previous spoiler and deleted it but after trying to install a new spoiler I am getting the following error message

I think something from my previous spoiler tag remained, I tried to delete everything connected to the spoiler from my Sources/Subs but the error message won't go away.
Could anyone advise me what to try from here? Thanks.
ETA I managed to delete enough to put the forum back online. Now I have an annoying spoiler code at the top of the forum. Does anyone know a quick easy way to get rid of it or do I have to do it manually in all the themes?
