That piece of code is added to the html after the closing HTML tag in the popup window of the quiz, that stops the quiz mod from working.
Find where and why this code is added and it will work.

I remove Sisyphus Mod(Sisyfus mod will restore entered text in Quick Reply form and Reply form, after browser crash/closing ).Quiz work good

I have installed this mod, created quizzes (all that are enabled) but no user can access them.  If they try to play the quiz, a popup appears saying "You have already played this quiz league over the maximum amount of times allowed for this round."  problem is no one has been able to play it.  What causes this error?

If you can provide a test account would be much easier to debug the issue. ;)
