How to set up a form like this through SMF

I would like to set up something similar to this on my site.

A form that a person could fill out and submit to me so I could help them via email.

What would be the best way to accomplish this?

Well, you could do something similar using the Custom Form mod to create new posts in a special board, or if you really wanted to go the whole nine yards, you could use SimpleDesk to set up a sort of helpdesk - which is what it sounds like you're trying to set up?

I have looked at SimpleDesk in the past and it seemed like overkill.

Would like to have a single page that anyone, (guest and members) could fill out and submit to me.
