Topic/Posting "Template"

Ive come up with an idea that perhaps someone could work on. I have specific boards that have different functions, for example my site is a clan site; and obviously I have a "Recruitment" board on there, at the moment in order for someone to apply for "membership" they have to copy and paste a "Template" in a sticky topic and post it in a new topic then add the required information to it. Is there a way so that a feature is added in the admin panel to have customized "templates" (ie I design how and what info I want to be readily shown) and pin them to specific boards so that the result is when a "recruitee" clicks the "New Topic" button the associated "template" is already part of the post ready and all the user has to do is fill in the required info. Ive searched the forums here and cant seem to find anything associated with it. Plus with this feature you could have multiple templates for multiple boards saving the need for copy/pasting from stickied topics and adding info to a new post and thus saving a little bit of time and hassle.

take a look at the custom form mod this as close to what you want we have

actually... I'm not sure "as close to" covers it...   the custom form mod:

seems to be EXACTLY what he is looking for.

hmmm ok so gave it a try but doesnt seem to be working tho ive installed it on the latest version of smf, just had to manually parse one line into my Admin.php because it had a problem doing it automatically. I created a form, but cant work out why the form isnt now automatically showing when i click "New Topic" in the relevant board. If its doesnt do this then, I cant find a drop menu or anything such as so that I can use the form in a post. Any help as to what I may be doing wrong?
