Poll Request

On my the forums I run for work, we often have folks who want to do what I would call a multi-level poll. In short it would something like below:

Option 1   []Yes  []No     (10% Yes / 80% No / 10% Abstain)
Option 2   []Yes  []No     (70% Yes / 30% No / 0% Abstain)

And allow for listing multiple items and then allow the users to vote on multiple items in one of three ways as shown above.

Anybody know of anything like this?
