If you check in the mambots/system folder, are the two SMF_header_iclude files there?

Both SMF_header_include.php and SMF_header_include.xml are there yes.

Reading back on the thread I also have nothing in the error logs or the SMF error log.

Then the issue is definitely the combination of Mambo and PHP 5.2.5.

My host is currently waiting for some comeback from Mambo on this issue.

If they come back with a resolution I will post it here.
