Ultimate Shoutbox

so i reinstalled smf and i installed the ultimate shoutbox.  it wont appear anywhere and i dont see an option to turn it on or off.  I use 1.1.10.

well i just decided to install another shoutbox but now i tried installing the file manager.  it wouldnt let me auto install it so i manually installed it.  now the profile part is broken.  i dont know how to install my backup.  it would be helpful if someone could give me the profile.php script from the default theme that has the file uploader script in it.  i would preffer a fast response because my users are getting impatient as its been broke all day today.
