Help with some errors please

Been trying to fix errors in my error log at sdx-developers.com all day

Here are some that are reoccuring

about that errors in index.template.php i'd say the $button array is just not filled completely.
as $button['active_button']  doesn't throw an error i'd just close forum for maintanence and var_dump that to get more information. (maybe var-dump whole $button array)

I believe it has to do with the menu buttons on the top of the forum. I just cant remember where those are located (which file)
