What is this in the Moderation log?...

I keep seeing this in the Moderation log...

Removed topic "Deleted - ID:00000" from "Account, Forum and Chat Room Issues"

I've replaced the actual number for the ID with "00000" - the number changes depending on what topic is affected.

Anyway, (1#) what would cause this?. There are no remove or delete permissions granted on the forum so it strikes me as odd that this would be showing up and (2#) where do I go in the ACP to disable this remove/delete ability if that is what's causing this?.
