A questionnaire/survey mod or work-around?

Hey SMF users, I'm not too sure if this is in the correct section, but I use 2.0 RC4 so this looked like the right place.

I currently help run a Gamecube forum and over the years we've gotten a lot of repeated questions concerning help for emulation. What we thought would have been a good idea was if there was some way that everytime a member posted for help in our "Emulation Help Section", they would have to fill out a questionnaire with some of the questions we've had to ask manually anyway. What this would do  is save us time and gets us helping them by getting to straight to the point.

Is there any modification or even some other work-around out there that could help me achieve this sort of system with our forum? Any help on this matter would be greatly appreciated.

you could look into the custom form mod that would paste the output to a post in that board. I am not sure if it has been upgraded to work with 2.0 yet though.
