Custom Profile Fields

I just installed SMF 2.0 RC5 and so far it has been AMAZING! There is one feature I can't figure out yet and it is related to profile fields. I am running a forum for my medical school and would like to enable some custom fields like:

Are you currently doing rotations? (checkbox)

And based on that checkbox if it is checked or not show other options such as:

What rotation are you enrolled in? (Drop down: Internal, Family, Surgery, Peds, Psych, OB/GYN)
What hospital are you at? (Drop down: with all hospitals based on rotation category student picked)

Is something of this nature possible? It would be a tremendous help if anyone can point me in the right direction. I want to avoid allowing students to input their hospital names because it will result in each user having something slightly different for the same location.

This isn't possible with a default installation. You will need to either hire a developer (Help Wanted (not for support)), or throw in a Mod Request so this is coded.
