Are you certain you entered a database name in the box?

No redirection... looks like it should work fine to me.  Again, are you certain you entered everything?  Is Settings.php changing at all?

The ad banner at the bottom won't help things, mind you.

How about Settings.php?  Did you check it?  Does editing it help?

You can use repair_settings.php to edit and view it. What is repair_settings.php?
