In the moderation center there is a log which lists all the warnings, which is a report. Is that what you want?

Moderation Center » Watched Members » Member

That shows a list of Watched Members, Warning Level, Posts, Last Login date, and Last Post date

I can't find where to change the word "watched" to "warned" ?

Something like:

$txt['warn_watch'] = 'Watched';

(From my memory. Could be subtly different)
