Duplicates in Recent Posts list

A member started a new thread with just one post but it has appeared 4 times in the recent posts list over the course of several minutes.

Anyone got any ideas on why this has happened and how to fix it?

Mods installed?
Url of your forum?

Url showing the thread that the user posted?

Did the user actually post 4 times?  Or maybe make 4 consecutive posts in the thread?
