How to integrate new php page with forum

So i have added a new php page, I have added to the menu next to Forum. However when I click it I want it to have the same theme and keep the menu up at the top. It just feels like its part of a different site, a little amateurish. How can i do this?

This mod will add an unlimited number of custom actions, configurable in Features and Options. There are 3 types, HTML, BBC, and PHP. They can be accesed by going to http://www.yoursite.com/smf/index.php?action=action name. Each action has an unlimited number of sub-actions, which can be accessed by going to http://www.yoursite.com/smf/index.php?action=action name;sa=sub-action name.

I've emboldened where you'll find the options.

yeah its not there

Yes it is, see the attachment.
If you don't have the option to create a new action from this sub menu, then I would respectfully suggest the mod failed to install correctly.
