Help with having a Form work in the forum

I am a  member of a gaming community playing WOT. Our forum needs an "apply to claws" form. I have tried the "code" tag and well as enabling HTML on the boards. Is there a way to get the coded from below to work on the Forum? Is there a Mod (I have looked) that I can use to recreate this form? Any assistance would be appreciated.

where are you trying to use this form? in a thread/post? you can not as it contains its own form. maybe the custom form mod may do what you need.

yup... this looks like a perfect thing for the custom form mod...

users fill out the form -- and the contents are then submitted into a post (which can be public or g=can be put into a board that only specific groups can see)

The Form will be a post for users to fill out. The end result when they click "Submit" is to send the filled out form to an email address not to a post. I guess that I can have it post to section that is only viewable to recruiter's and game clan officers.

Thanks guys for the info. I will look into the mod and see if it will  do as needed
