Moderator getting Banned message even tho she's in good standing

I have a moderator who is getting a specific banned message i only used on one member a couple of years ago.  I can not find any triggers that should trigger that message.

beginning;
i banned a member for 'excessive sarcasm' and used those words exactly.  I did an email, username & ip ban

last week a member who has moderator privileges in one or two areas reported seeing the same message when trying to log on via her phone.  I removed the ip ban on the first bad banned member.

the Mod in good standing reported today she's still getting the 'banned message for 'excessive sarcasm' '

i can not figure it out why she is not being able to log on
the ban log show's the ip she's trying from-- that is *not* on the ban trigger list
nor is her email address or username on the ban trigger list
her account does not show any flags and is in good standing.

i am tempted to delete **all** ip's from the ban trigger list.

Ip is a bad way to ban anyway. ..

Ask her to clear her browser cache

it's happening only on her phone & not her desktop.

i just asked her if she's turned her phone completely off recently  - <palmplant> for not thinking of it sooner

i saw the thread on 'why not to use ip ban' -- it' makes sense.
