Creating a form where the user can select the board

Hi people

I would very much like to create a single form which users can submit a topic and select the board from a selection list.

My plan is to have this available from an external page of recent topics which I've creating using SSI.

So the form would have ...

Name
Email
Subject
Classification (for example) which has a drop down list of the boards which are available as defined by admin
Message

Any clues appreciated.  My php knowledge is .. dodgy !!!
