Members not receiving activation e-mail

Was running 2.0.8, just updated to 2.0.11

A couple of people have complained that they did not receive an activation e-mail after registering.  How can I determine if these are isolated cases or if there is a bug in the activation system?  And if there is a bug, what can I do about it?

2.  I found the two in the Awaiting Activation list and activated them manually.  I never go to that list and was shocked to find 2,000 (!!!)  awaiting activation.  What the hell are  those, spambots that don't/can't respond to the activation link?  Or what?
