Is there a mod that lets users generate a report?

Hi all

I'm wondering if someone might be able to help me.

I'm looking for a mod of some sort(if it exists) that allows a user to generate a report. Kinda like SMF Articles but there needs to be the option to have more fields for the user to fill in before submitting the report/article. Once the reports are submitted they are listed under each other with the option to view each report in more detail on another page.

Is there a mod out there like this?

Do you mean, something like the custom form mod, which allows you to create a form with various fields and drop that into a post when it is submitted?

Yes something like Custom Forms Mod but that mod doesn't work correctly on my forum, the data doesn't get displayed on the post it generates, for some reason ...  also what would be ideal in my situation is for a user to be able to attach images to their report as well
