Issue with email verification for members after sign-up?

I have tried to make test accounts and for some reason their seems to be minor discrepancies with the log in system for my forum. I found that the email verification to activate new users accounts after sign-up would never send. as well as other captcha issues. I was wondering if anyone had any advice with these type of issues?

Provided notifications turned on, some quick ideas:

Send yourself a pm.
Activate notify on a bunch of active topics.
Make a newsletter, and send only to yourself (admin membergroup).
Register a new account (need another email address), see if you receive the activation email, account also good for more testing of notify\pm notifications, permissions etc.

Enable Mail Queue, set to 1 pr minute, do some mail-generating activities, see if 1: showing up in queue, 2: queue emptying.
