Uploading Photo Kills Thread?

I'm having an issue on our forum.  If I upload a photo to a thread, sometimes(not always), the thread becomes unviewable to any members signed in.  All you see is a white blank screen.  If you're a guest, you can read the thread, b/c I've disabled picture viewing for anyone not logged in.

It would seem that while the picture has uploaded successfully, its corrupting something on that post.

I'm not really sure what's causing it, I can say that if I try to post it again, it will say that it that post already exists.  However, again, if I'm logged in, that thread is dead & unviewable.  Any one have any ideas how to fix a thread that I can't view?

We're SMF 2.0.11, with the following mods:
