Referral message to add to registration - how do I do this?

Hi

We would like to add an extra field to the registration so that when a new person registers, they have to fill in a text field saying which member invited them

It would be something like:

                                 --------------------
Who referred you:   |    fred sent me      |
                                --------------------

It has to be mandatory

It has to be free text of say 40 characters or so

It has to be visible to the Administrator when they are approving new members via member approval screen. 
We have membership approval in place, so when the admin goes to approve new members they will also see what was entered in this field.

We also want to use this to reward existing members for recruiting new members to the forum

I think I know how to add a free text field to the registration template, but what else do I have to do to make this happen correctly and show up on the admin approval screen?
