User Feedback Mod (Paid!)

There are quite a few people interested in a user feedback mod.  It would be similar to what ebay has for rating transactions.

I thought I would dig this up as I havent heard much about it in sometime, and alot of people were interested.  Based on posts this is a paid mod, let me know how much etc.
