I want something like this:

Except make both it so you can define if its a text field or dropdown box. and you can change what the name of it is. And if its a dropdown box you can add/remove choices. Also you can have as many of them as you want, and most inportantly you can choose what forums it is enabled in.
Can someone please make this?
