How can I add the "message box" at the top (as shown on this forum) which shows PMs, unread posts, and new replies?

Is that a mod? I couldn't find it in the available packages?

That`s default on all SMF installs.

On my forum www.evangelismforum.com it doesn't show - is there a setting I need to change?

If you want a header image that big, you will have to change the code so that it is in a separate table row. It's just going right over the top of the rest.
