I have recently set up a new smf forum (Version 1.1.3 ; mail via php) and noticed straight from the beginning, that the registration approval notification email is never sent to the newly approved member (member approval set active). The new member receives a Welcome email when registering but never the promised approval notification.
Following bit from the error log probably addresses the problem as well:

Apart from this the email notifications seem to work on and off...

There might be the problem. Contact your host about any e-mail sending restrictions, as well as any anti-spam filters they may have set up on your account. Point them to this topic too.
