I just had a new member join my site and when I searched his email he came up on many SMF boards. His Member ID is "Peace890" and his email is "[email protected]"

I find this a bit suspicious that someone can register so many times on different boards around the same time.

Is this a security issue?

I do have image verification turned on.

You have it set to its simplest setting.

I use advanced captcha and are you human mods on my fora.
