Hi there!
I volunteer my time and so on for an organization and I have been asked to create a form for signing up more volunteers. I have a copy that people can print out and mail-in, but I was asked to create a form that can be filled right in one of the posts and submitted right there. I am wondering if anyone knows a form building service or a way that I can do it? I have build one out of flash, but the way the results of the form come out is hard to decipher and I need the form to look like the original. I am attaching a screenshot of the form looks like.

Is PDF an option?
Not really. The guy wants to make sure that everyone can fill out the form right online and be able to e-mail it in right away. PDF would mean that people would have to mail it in and he is afraid that people wouldn't want to do that.
Nah - server extensions enable PDF via the GD-LIB and more. The "more" is for what I don't know but I know that there are extensions available to display PDF using the GD-library with Apache. I am sure there are piles of ways that I don't know. Your goal, as I understand it, is to have it in a displayed format easily... Your other option is to parse the information via XML to a template that is interpreted at the client level which is what I'd recommend if you had the time and code prowess but you probably don't have the first and if you had the second you'd have exceeded my knowledge. The problem with that is that you then must find a universal way to display the data that works for all OSes whilst a single PDF file SHOULD display just fine in any modern OS with the appropriate software installed.
