successfully upgraded to 1.1.12 but index.php?action=packages from the admin panel is failing to open, its showing error 500, what must have gone wrong?

usually a 500+ error message is server side, any messages in your error log? anything in your server error log?
What method did you use to upgrade? As Illori stated this is normally a server error. Be sure and let us know if there is anything in your forum error log as well. Here is some additional information on the error.
