Have been doing a really good job keeping that error log clean, just had this show up twice over the past couple days. I tried a bunch of google searches, but can't locate what mod may use the 'to_name' hook.

The error is easy to understand... The code is using a 'to_name' column which isn't being retrieved by the query.
The culprit is this

the error seems related to the Slash through Banned Usernames mod, the support topic seems to say the mod is pretty buggy. i would recommend uninstalling it. to_name is not defined in the query before it is used in this location from what i can see that is causing the error.
