what mods do you have installed and is this for smf v1.1? if so i'd highly suggest updating to 2.0.17 for security reasons and also php versions....

I know for me on a 2.0.x it was the mod Topic Author (https://custom.simplemachines.org/mods/?mod=3485) for some strange reason with that installed my captcha stopped working, uninstalled it and my captcha was fine....

so well....if you don't have that mod then you'll need to play eeny meeny miney mo until you figure out which mod is causing it.

Core captcha verification is pretty much useless.  You should upgrade to 2.0.17, 1.1 branch went eol a while ago and is no longer supported.

So you're running 2.0.17.  You should be using verification questions for registration, not captcha.

6 questions is not enough...

you should have 20-30 questions in the queue (ask 1-2 each registration)
and you will also need to change your questions approximately every year or two.
