Title: Fixing "Sorry, but you've reached your login attempts threshold..."

We had a user who simply could not login with the dreaded message "Sorry, but you've reached your login attempts threshold for account ******. Please wait 30 seconds and try again later." Since they verified they could do it from another browser, the obvious culprit was an issue with their SMF cookie. Also, in the error log of our 2.1.3 instance I also saw the message "8: Trying to access array offset on value of type null" at exactly the same instance of the failed login.

Now the problem was resolved by our user removing the SMF cookie from their Chrome browser so that it was correctly regenerated, but I thought I would post this in case others had the same issue and also because I saw this in the release notes of SMF 2.1.3 which I had just also updated:

Since the message was exactly the same, I was wondering if there still is a lingering issue in 2.1.3

I would add that several other users had no problems logging on and off so this was just isolated to a single user afaik.

It was fixed,  not lingering
