From your screenshot your using a custom theme you will have to install the mod manually on that theme if you go to the mod page on the right you select your version of smf and parse it this will tell you all the edits you need to make you only have to make the edits for the files in your theme.

I'm not really sure what the cause of it is, could be the size of the advert banner, try a smaller sized advert perhaps. Or go to the authors support forum.
