Guild Application posting to Forums.

Hello All,

Fist let me say I am not sure if this is the correct board.  If not I apologize in advance.

I am looking for a mod / add-in / method for setting up a web page with fields that can be filled out by an anonymous user and then submitted.  The submission would then post to my Forums for review by the other administrators.  I have looked over the mods site and did not see anything that would do this.  I know I have done this before *shiver* PhpBB so I'm certain there is a way to do it in SMF.  Can anyone point me in the right direction?
