Undefined index: forum_name2

My SMF site has been working fine until today. I have made no changes at all, and no one has even posted to the site in the last week, so this problem has appeared out of knowhere!

Problem is this....
all pages now show this error message at top of page, plus all formatting has been lost, so page backgrounds are white and font size is large. Looks horrible!

Notice: Undefined index: forum_name2 in /files/home1/joebloggs/smf/Sources/Load.php(1090) : eval()'d code on line 148

..where joebloggs in the name of the SMF site.

Does anyone know why this has suddenly happened?

Update: I have managed to get my formatting back by changing back to the Default SMF Theme. Still have no idea why my custom theme suddendly decided not to work.

I still have that error message at the top of each page though.

Sounds like a "2" was added somewhere on line 148 of a template or language file; it should just be forum_name.

There may be other problems...

Try uploading a fresh copy of index.template.php to whatever theme you're using.
