Just try that PHP, it works allmost everytime. Leave those SMTP boxes empty.

you should ask your host which method(s) they use for sending email as it is host specific in some cases.  Some allow php's sendmail, some only allow email sent through SMTP.

Generally speaking, the email address that you are sending from MUST actually exist on the server or it will be disallowed through.

Only your host can debug why emails are being blocked.
