Shout Box error for SMF 2.0 Beta 4

I followed these link Instructions for edits, i did everything but i am getting error

I attached the file of my error screen shot, i hope i did edits correctly but i don't know why i am getting error. and also i attached my index.templet.php

You should ask for mod related support, in the support topic for that particular mod. In this case, you can get find the support topic by clicking on the 'Support and comments for this mod' link on the download page of that mod.

Have you tried asking for support in the Mods support topic? The Mod Author is the best person to help you with a particular mod and any issues with it.
