Global Moderator suddenly can't add attachments

This problem occured out of the blue the other day.

I have a Global Moderator who has his own board where he posts music files to share with the members of our site (ThreeStooges.net.)  He's been adding attachments with no problems for months now.  Last week, I decided to increase the amount of space allotted for attachments on the site, and I increased the size per file and the number of files allowed per post.

Since then he has been unable to add any attachments at all.  Whenever he tries, he gets the following error message:

LainaaSorry! There is already an attachment with the same filename as the one you tried to upload. Please rename the file and try again.

I've checked his permissions several times, and they're all set correctly.  I am able to add attachments to his topics as the Administrator, and I opened a different browser, logged in as my alter-ego (which is also set as a Global Moderator), and I was able to add attachments to his topics as that user as well.

I've searched here to see if someone has run into this problem, but I didn't find anything.  Does anyone have any suggestions?

I found part of the problem - there were 42 entries in the smf_attachments table with both the ID_MSG and ID_MEMBER fields set to 0.  When I looked at the filenames for those entries I realized there were all from my member's former posts, which he had previously deleted.  (He leaves a topic with a group of files up on the board for a few weeks, then deletes it and posts a new one.

I am guessing that he simply deletes the topic.  Am I right in guessing that when he does that it does not delete the asctual files or entries in the smf_attachments table?  If that's the case, then I need to find a fix for that; when a post with attachments get deleted, the files and smf_attachments entries should get deleted as well.

Sorry for taking so long to respond.  Deleting topics from within SMF (as compared to manually in the database) does indeed delete the files and rows from the attachments table.  I'm not sure why your's wouldn't be doing this.
