Probably a conflict issue with the SEO mod you're using.  If you turn off those features do you get the same results?

Did it ever work?  If so what changes were made from when it was to wasn't?  List of mods installed.  Does it occur on the default core theme?

I've done a fresh install this evening and it seems to have fixed the issue
