Using repair_settings.php is easy: simply upload the file to the root directory of your forum with FTP and then access the page in your browser. Once you have finished, remember to delete the file or else you will leave your forum vulnerable!

Means
1.upload the file to root directory
2.Run the file using path like ..   www.yousitename.com/repair_settings.php
3.See the paths and check for recommended values shown in repair_settings.
4.after being done remove the repair_settings file from your ftp.

That's always assuming that index.template.php actually exists.

Have a look in the SMF default theme's directory, to see if it's there.

If it is, make sure it's read-enabled.

When you download the file, it might be named html, for some reason. You have to rename it to php.

Hang on: it has to go in the forum root. The same folder that holds Settings.php. This is not necessarily the top ftp directory.
