Undefined Index Errors on 4 Lines {36, 37, 456, 671}

I am running 1.1.13 and I noticed this in the error report about 1 trillon times.

8: Use of undefined constant Forum - assumed 'Forum'

What needs to be done to correct this error for Line 36, 37, 456 and 671?

Have you made any edits to the theme files recently?.
It maybe just the theme it's self. An old theme.

please install http://custom.simplemachines.org/mods/index.php?mod=2054 and enable it, then post the new errors without eval in them.

I enabled the Mod and this is what the error log shows now...

I did try to install a Rep system about a year ago, but it was uninstalled and deleted. Could that be what is causing the issue?

Hence the pos_rep and neg_rep.

What would I need to do to correct this issue since it has been uninstalled and deleted over a year ago?

first thing would be to parse the mod and manually try to remove the code that is left. basically doing the reverse of this Manual Installation of Mods
