Header of blocks help

this is a fresh install of a new forums, so i was almost going to just reinstall but id figure if someone could narrow down where to check for the issue at. The headers on all of my blocks have a small section on their left side which doesnt have the appropriate background as shown in this pic, any help is appreciated.
