Fatal Error in Signature Field in Posts and Profiles

Hello Folks,
I have been using SMF for a while.  I recently upgraded from 1.1.11 up to 1.1.14 and I am seeing errors in the posts of forum members and in their profiles that looks like the following:

Fatal error: require_once() [function.require]: Failed opening required '/home/hxcchris/public_html/forum/Sources/CustomProfile.php' (include_path='.:/usr/lib64/php:/usr/lib/php') in /home1/hxcchris/public_html/forum/Sources/Load.php(1733) : eval()'d code on line 364

I have a custom theme, so this might be part of the problem, but not exactly sure.  Any help would be greatly appreciated.

Unfortunately, that "eval" thing could be masking the true error.

Can you install this mod http://custom.simplemachines.org/mods/index.php?mod=2054 and enable it?

Then, we should see the true error.
